Description:

Preclot 75 contains Clopidogrel, an antiplatelet medication used to prevent blood clots (thrombi) due to atherothrombosis, which can lead to serious cardiovascular events such as stroke, heart attack (myocardial infarction), and peripheral arterial disease. It is particularly beneficial for patients with atherosclerosis and those who have undergone percutaneous coronary intervention (PCI) to manage in-stent restenosis.

  • Clopidogrel works by preventing platelets in your blood from clumping together. This action reduces the likelihood of forming harmful blood clots, which can cause blockages in the arteries, leading to heart attacks or strokes.
  • Do not take this medication if you are allergic to Clopidogrel.
  • Avoid it if you have any condition causing active bleeding, such as a stomach ulcer or bleeding within the brain.
  • Severe liver disease is also a contraindication for this medication.

How does Preclot 75mg Tablet work?

Indication of Preclot 75mg Tablet

Pharmacology

Clopidogrel is a prodrug. It inhibits platelet activation and aggregation through the irreversible binding of its active metabolite to the P2Y12 class of ADP receptors on platelets. Dose-dependent inhibition of platelet aggregation can be seen 2 hours after single oral doses. Repeated doses of 75 mg per day inhibit ADP-induced platelet aggregation on the first day, and inhibition reaches a steady state between Day 3 and Day 7.

Adult Dose for Preclot 75:

Oral Prophylaxis of Thromboembolic Disorders, Recent MI, Stroke, or Established Peripheral Arterial Disease, Coronary Artery Disease:

  • Adult: 75 mg once daily.

Acute Coronary Syndrome (ACS):

ST-Elevation Myocardial Infarction (STEMI):

  • Adults under 75 years: 300 mg loading dose followed by 75 mg once daily, typically for 14 days up to 12 months, if there is no significant bleeding.
  • Concomitant Therapy: Administer in combination with aspirin 75-325 mg once daily, with or without thrombolytics.
  • Adults 75 years and older: No loading dose; just 75 mg once daily for 14 days up to 12 months, depending on the risk of bleeding.

Unstable Angina (UA) or Non-ST-Elevation Myocardial Infarction (NSTEMI):

  • Adult: Initially, a 300 mg loading dose followed by 75 mg once daily for up to 12 months. This should be combined with aspirin.

Hepatic Impairment:

  • Use with caution due to limited experience in this population.

Renal Dose:

  • Renal Impairment:
    • Dose adjustment is not necessary.
Children

Clopidogrel is not recommended for children.

Administration of Preclot 75mg Tablet
  • Take Preclot 75 exactly as prescribed by your doctor.
  • Swallow the tablet whole; do not chew, crush, or break it.
  • It can be taken with or without food, but try to take it at the same time every day for consistency.
Interaction

Drug-Drug Interactions:

  • Blood Thinners: Combining with other anticoagulants like warfarin or heparin increases the risk of bleeding.

  • Gout Medications: Probenecid interaction can increase the risk of adverse effects.
  • Anti-Epilepsy Drugs: Medications like valproate or phenytoin may have increased blood levels and toxicity.
  • Anti-Inflammatory Drugs: Ibuprofen and other NSAIDs can increase the risk of gastrointestinal bleeding.
  • Stomach Upset Medications: Omeprazole can reduce the effectiveness of clopidogrel.
  • Antifungals: Fluconazole can inhibit clopidogrel metabolism.
  • HIV Drugs: Ritonavir and cobicistat may reduce the effectiveness of clopidogrel.

Drug-Food Interaction: Limit alcohol intake as it can raise blood pressure and increase the risk of heart disease. Also, avoid fatty foods and maintain a healthy diet to control cholesterol and triglyceride levels.

Drug-Disease Interaction: Inform your doctor if you have a history of allergic reactions, heart disease, active bleeding disorders, asthma, ischaemic stroke, liver, or kidney disease, as these conditions can increase the risk of side effects or alter the effectiveness of Preclot 75.

Contraindications
  • Contraindicated in patients with known hypersensitivity to clopidogrel or any component of the product.
  • Contraindicated in conditions such as peptic ulcer or intracranial hemorrhage.
  • Contraindicated when administered within 7 days after myocardial infarction (MI) or ischemic stroke.
  • Use with caution due to bleeding risks.
  • Contraindicated during breastfeeding.
Pregnancy & Lactation

Preclot 75 is not recommended for breastfeeding mothers since the drugs can pass into breast milk and potentially harm the nursing infant. Discuss alternative treatments with your doctor if you are breastfeeding.
